Refill Requests: Please check refills on your medicine bottle first and if it says “zero” then contact your pharmacy and they will contact us directly to arrange.


About Your Appointment

  • Bring up-to-date ID, insurance, and all medicines/supplements (or a complete list) to every visit.
  • Visits are typically scheduled in 20 minute increments but may be shorter or longer depending on multiple factors
  • Plan to arrive 20 minutes prior to your appointment (for established patients) or 40 minutes prior (for new patients).
  • Before your visit, be prepared with all questions and health issues to discuss.
  • Walk-in visits are available on a first-come first-serve basis. You are advised to call the office prior to coming to gauge our schedule for that day. We will do our best to fit you in with respect/priority to patients who are already scheduled.
  • We charge a $35 fee for missed appointments when there is a failure to notify us ahead of time (same day cancellations/rescheduling may be allowed).
  • Bring a method of payment to every visit (cash, check, and all major credit cards accepted). Copays are due at time of service.
  • Please understand: we do our best to stay on-time/on-schedule, however, sometimes complex/urgent/emergent situations occur and we need to spend more time with a patient than is anticipated. Our patients’ health is our top priority. Please remember you will receive the same attention when your time comes to see the doctor. We know your time is valuable and we thank you for your patience and understanding.
  • For health emergencies, you may contact our office but strongly consider going to the nearest urgent care or emergency room immediately without delay.
